Antibiotic of biosynthetic penicillin group. Has a bactericidal effect by inhibiting microorganisms’ cell wall synthesis.
Most effective against gram-positive bacteria: Staphylococcus spp., Streptococcus spp. (incl. Streptococcus pneumonia), Corynebacterium diphtheriae, Bacillus anthracis; against gram-negative bacteria: Neisseria meningitides, Neisseria gonorrhea; against anaerobic spore-forming rods, as well as Actinomyces spp., Spirochaetaceae.
The strains of Staphylococcus spp., which produce penicillinase, are benzylpenicillin-resistant. Dissolved in an acidic medium.
Benzylpenicillin sodium salt compared to potassium salt has longer action duration.
After i.m. injection it is quickly absorbed from the injection site. Widely distributed in tissues and body fluids. Benzylpenicillin penetrates well through the placenta. BBB in case of meninx inflammation. T½ - 30 minutes. Excreted with urine.
Treatment of diseases caused by benzylpenicillin susceptible microorganisms, сroupous and focal pneumonia, pleural empyema, sepsis, septicemia, ichorrhemia, acute and subacute septic endocarditis, meningitis, acute chronic osteomyelitis, urinary tract infections and bile ducts infections, quinsy, skin purulent infections, soft tissue and mucosal infections, erysipelas, diphtheria, scarlet fever, malignant pustule, actinomycosis, treatment of suppurative inflammations in obstetrics and gynecology, ENT diseases, eye diseases, gonorrhea, blenorrhea, syphilis.
Hypersensitivity to benzylpenicillin and other drugs of penicillins and cephalosporins group. Endolumbar administration is contraindicated to patients with epilepsy.
Dosage and Administration
In moderate infection: 250,000-500,000 units intramuscularly and intravenously; daily dose: 1,000,000-2,000,000 units. In severe infections (sepsis, septic endocarditis, meningitis): 10,000,000-200,000,000 units/day; In progressive emphysematous necrosis: 40,000,000-60,000,000 units.
Daily dose of children under 1 year: 50,000-100,000 units per 1 kg of body weight, of children above 1 year: 50,000 units/(kg per day). In severe infections (meningitis, sepsis, severe forms of acute pneumonia) daily dose can be increased to 200,000-300,000 units/kg; for the treatment of life-threatening conditions: 500,000 units/kg.
Daily dose is divided into 4-6 injections.
In intravenous injection dissolve 1,000,000-2,000,000 U in 5-10 ml of water for injection or isotonic sodium chloride solution, inject 1-2 times a day in combination with intramuscular injections.
In intravenous drip injection dilute 2,000,000-5,000,000 units in 100-200 ml of isotonic sodium chloride solution or 5-10% glucose solution.
Depending on disease, 5,000-10,000 units are injected to adults, 2,000 to 5,000 units – to children. Dilute the drug at the rate of 1,000 units per 1 ml.
Used subcutaneously for puncturing infiltrates (100,000-200,000 units per 1ml of 0.25-0.5% novocaine solution).
The solutions are used immediately after preparation without adding other drugs.
